Effective January 1, 2009 the deductible will be $300 per person or $600 per family.  The insurance payment for PPO will be at 80%.

Medical coverage is through Premera Blue Cross.  There is a $200 deductible per person or $400 per family.  Your insurance is a preferred provider organization (PPO).  That is there are contracted rates with a network of healthcare providers and the insurance payment is at 80% of the contracted rate.  If you choose not to use a PPO, the insurance payment is at 70%.  You should receive a medical card within the first two weeks of eligibility.  For more information refer to your Health & Welfare Booklet or visit www.premera.com.  You may also contact Premera at 1-800-722-1471.

Dental Coverage

Vision Coverage

Prescription Drug Coverage

Your prescription Drug insurance provider is MEDCO.  You pay a co-insurance fee/out-of-pocket expense.  You should receive your prescription drug card and information packet within the first two weeks of eligibility.  If you need to use your benefit and have not received a card, you will need to call member services 1-800-309-7139.
